As of January 1, 2025, the Patient and Caregiver Registry Portal is open for applications. You can access it by clicking on the link below. Applicants must complete an applicant signature page and upload it to the registration portal. The forms are available here. ​

​​The Program has issued a Qualified Patient & Designated Caregiver Guide​​​. This guide will assist individuals interested in becoming cardholders in the Commonwealth of Kentucky with applying for a medical cannabis card. The Guide will walk applicants through the steps to register, including the required information and documents they will need to complete the card application process. ​​​​You can find additional resources here.​
