The Office has created this page to assist cardholders by putting various cardholder resources on one page. New and updated information will be added to this page on an ongoing basis. 


How to Print Your Medical Cannabis Card

Medical cannabis cards provided by the Office are digital cards that can be printed by the cardholder. Digital copies of the card saved on your phone will be acceptable to use at dispensaries, but cardholders are encouraged to have a paper copy of their card if possible. ​

To view and print a copy of your medical cannabis card from the patient registry:    

  1. Go to your online account with the patient registry, where you applied for the card, here​
  2. Once logged in, Click License Dashboard.   
  3. Then Click “Print Digital Card” on the right hand side.   
  4. Select the patient number and download the card to print. ​​

How to Submit a Patient Update

You may submit Patient Updates to the Office through your account in the patient registry (where you applied for a card). To submit a Patient Update, which includes things like your profile information, contact information, and updated written certification, please use the following steps:   

  1. Click “+Click New Application” on the left-hand side of the patient registry portal (https://ky-chfs-public.nls.egov.com/login)  
  2. Select “Patient” 
  3. Select “Patient Update” 
  4. Then select your license number in the drop down menu and choose what needs to be updated.   

The Office will review the submitted Patient Update, and once approved, the changes will be implemented to your account.
