​​Reporting to the Office

There is no automated notification to the Office if a cardholder has been charged or convicted. Agencies should notify the Office of any violations so the Office can suspend or revoke the registration of a patient or caregiver who: 

Report violations via email to kymedcanreporting@ky.gov

Please provide the individual's name, DOB, Patient ID (if applicable), and any pertinent details of the violation (date of occurrence, case number, etc.).​
